Before your loved one can be placed in a skilled nursing facility, the State of New York law requires that a PRI assessment and SCREEN assessment must be done. We provide services in Staten Island, Brooklyn, Queens, Manhattan and Bronx.
The (PRI) Patient Review Instrument is a clinical tool that helps to determine how much assistance your loved one will need in the skilled nursing facility. The PRI is used to assess the physical, medical, and cognitive status of potential skilled nursing facility residents, as well as its purpose is to document certain services that they may receive. The PRI can only be done by a Registered Nurse who is certified as a PRI Assessor.
The SCREEN assessment is a tool that is used for 2 purposes; The first purpose of the screen is to determine the person’s ability to be cared for in a setting other than a skilled nursing facility. The second purpose of the SCREEN is to assess persons being recommended for skilled nursing facility placement for possible mental illness(MI), and/or mental retardation/ developmental disability(MR/DD), the purpose of this is to assure that seriously mentally ill and mentally retarded /developmentally disabled (MR/DD) persons are appropriately placed in order to provide for their treatment needs. A SCREEN can be completed by a SCREEN certified RN, social worker or other health care provider who is responsible for discharge planning and Skilled Nursing Facility placement.
